Below is a series of Activity Sampling Cards provided by forces for your reference to help you in the design of your cards.
Activity Sampling Cards – Examples
FormMaster Cards
General FormMaster AS card 1 - Used for majority of operational officers. This card meets the minimum Home Office requirements.
Traffic AS card - Built specifically for the Traffic section. This card includes the optional breakdown code ND (Traffic Other).
Specials AS card – This card has slight changes ie absence of paid/unpaid overtime and change of ranks.
Airport AS card - Caters for airport activities ie local codes which map into Home Office codes. As many general codes are not applicable.

FormMaster Borough Supervisors’ card - Kindly provided by the Metropolitan Police. Please contact Kevin Bush for more information.
Cardiff TELEform Cards
General Cardiff AS card - Generic card for general use. Kindly provided by Humberside police. Please contact Ian Fleming for more information.
Anti-vice card - This and the following Cardiff TELEform cards have been tailored to meet the needs of different force departments. Incidents/Activities that are not relevant to the department have been removed and some extra specific Incidents/ Activities added where needed.
SOCO card (Scenes of Crime Officer card) - These cards have kindly been provided by Nottinghamshire police. Please contact Sarah Hooper for more information.

General Formic card - Generic Formic card for general use. Kindly provided by Humberside police. Please contact Ian Fleming for more information.
